- Full or disconnected voicemail
- Refusing follow-up care due to abnormal lab results
- Abnormal results can only be discussed in person
- Making appointments and not completing the online check in
- Cancelling/rescheduling one day before your visit
- Being confrontational, hanging up the phone, and/or using profanity

* Once they add up to 3 VIOLATIONS patient is discharged - No Exceptions
* These policies do not reset at the end of each calendar year
